BOSTON – PatientPing, a health technology company that connects providers to coordinate patient care, is collaborating with HCR ManorCare, a provider of short-term, post-acute services and longer-term care, to enable real-time patient care coordination. With the PatientPing platform, HCR ManorCare will proactively monitor patient populations and access data to inform care decisions. “With PatientPing, we’re able to gain valuable patient insights that help providers to reduce hospital readmissions and ease our patients’ transitions from one care setting to the next,” said Marty Grabijas, vice president of sales and marketing at HCR ManorCare, in a statement. HCR ManorCare has a network of nearly 500 skilled nursing and rehabilitation centers, memory care communities, assisted living facilities, outpatient rehabilitation clinics, and hospice and home health care agencies. With this collaboration, PatientPing will accelerate its expansion from nearly 20 states to the remainder of the U.S., with the aim of creating a national health care system in which providers can easily come together to ensure medical information is shared and care is coordinated, regardless of where a patient chooses to receive care. “HCR ManorCare’s expansive network helps us more rapidly grow our national footprint so providers can more easily coordinate care across the healthcare continuum,” said Jay Desai, CEO of PatientPing in a statement.
