MANCHESTER, N.H. – The Visiting Nurse Association of Manchester and Southern New Hampshire has added a patient monitoring solution from Health Recovery Solutions to reduce readmissions for high-risk patients and improve quality of care. “The HRS solution is helping us add another level of care and support for our patients and empowering our staff,” said Carla Braveman, vice president, Home and Community Services, VNA of Manchester and Southern NH, in a statement. The HRS platform combines patient engagement software with biometric monitoring and mobile apps. All data captured through the devices is automatically transmitted back to the clinical team in seconds, allowing real-time interventions. The solution also triggers alerts for patients at high risk of medical emergencies. The agency is providing their patients with the HRS software on tablets and Bluetooth biometric devices.
