TEMPE, Ariz. – Life365, a digital health platform provider, is teaming with American Medical Response to deliver remote care management services for special patient populations across the country.
Life365’s digital health platform and AMR’s Mobile Integrated Healthcare program allow health plans, health care systems and local communities to leverage innovative technologies and the expertise of more than 35,000 clinicians and support staff. “The home is becoming an extension of the community health system, where organizations like AMR can provide personalized care based on the needs of the patient, and technology can be used to provide continuous feedback to care teams for better patient outcomes,” said Kent Dicks, CEO of Life 365, in a statement.
Life365 will support AMR’s Integrated Healthcare program with proactive health monitoring tools used in the home like connected medical devices, two-way video, patient education resources, health surveys, medication reminders, digital therapeutics and other tools to improve patient engagement and enable earlier interventions for health issues.
The initiative allows Life365 to manage connectivity and technology while AMR’s care professionals work with the patient and the patient’s care team to focus on improving the overall experience and health outcomes of the populations they serve.
“Our goal is to improve the patient experience, reduce the overall cost of care and improve the access to care for patients across the U.S,” said Sean Burton, national director of integrated healthcare at American Medical Response. “We see the potential in working with Life365 and believe it will help us grow our remote care monitoring program and offer an unmatched level of service to our customers.”
